What is MENTHIS

Your NGS report is data. MENTHIS turns it into a treatment decision.

MENTHIS is the decision engine behind every NgsAdvisory report. It brings together tumor genomics, treatment history, resistance mechanisms, co-mutations and current clinical evidence to produce a prioritized molecular treatment strategy your treating physician can use directly.

§ 01 · Definition

MENTHIS is what?

The sequencing lab tells you the mutations exist. MENTHIS determines what those mutations mean for this patient.

MENTHIS is a clinical decision-support engine that interprets the molecular profile as a biological system, not a list of mutations. Raw genomic complexity goes in; the patient's diagnosis, treatment history, co-mutations, resistance biology and current evidence hierarchy are integrated into a single structured output — structured clinical intelligence comes out.

§ 02 · Molecular intelligence

MENTHIS evaluates what.

01

Driver biology

Is the alteration genuinely oncogenic and biologically determinative? Driver is distinguished from passenger mutation.

02

Targetability

Is there a treatment option with clinically meaningful evidence — and if not, why not?

03

Evidence ranking

Treatment options are ranked in a structured way using ESCAT and similar accepted evidence hierarchies.

04

Co-mutation analysis

Do additional genomic alterations change treatment sensitivity or prognosis?

05

Resistance biology

Are there intrinsic or acquired resistance mechanisms? Possible bypass pathways are assessed.

06

Germline flag

Does the finding point to a hereditary cancer predisposition requiring confirmatory germline testing?

07

CHIP assessment

Does a variant on a blood-based test reflect the tumor, or clonal hematopoiesis? A critical distinction.

08

Testing-gap detection

Have RNA sequencing, fusion analysis, MSI/MMR, TMB, CNV and tumor-specific biomarkers been assessed?

§ 03 · Scope, honestly

What it does and does not do.

What it does
Turns molecular results produced by qualified laboratories into a treatment strategy a physician can act on
Distinguishes driver from passenger mutations and assesses clinical significance
Ranks treatment options against ESCAT and similar evidence hierarchies
Systematically flags co-mutations, resistance mechanisms, and possible testing gaps
Offers clinical-trial matching by molecular and clinical eligibility where applicable
Flags potential germline findings for confirmation, without making a diagnosis
What it does not do
Does not perform sequencing — it structures the meaning of existing molecular results
Does not replace the treating physician, the pathology diagnosis, or tumor-board review
Does not make a germline diagnosis — definitive findings require confirmatory testing and genetic counseling
Does not present an unapproved option as equivalent to a guideline-backed one
Is not a medical device and does not replace clinical judgment
Does not substitute for local regulatory and reimbursement requirements

MENTHIS provides expert molecular-oncology interpretation and clinical decision support. It does not replace the pathology diagnosis, the judgment of your treating physician, or multidisciplinary tumor-board review.

§ 04 · Decision architecture

Six layers, into a single treatment decision.

Every meaningful molecular treatment decision requires a systematic evaluation across multiple clinical dimensions. MENTHIS processes these layers in sequence, from patient to treatment decision.

01 Patient The clinical baseline: diagnosis, stage, performance status, comorbidities.
02 Clinical history Prior treatments, responses, progression pattern, and toxicity profile.
03 Molecular profile The full spectrum of NGS findings: SNV, indel, CNV, fusion, MSI, TMB, and any available RNA data.
04 Biological interpretation Driver hierarchy, pathway activation, co-mutation effects, and resistance-mechanism assessment.
05 Evidence layer Assessment of guidelines, regulatory approvals, and trials alongside prospective, retrospective, and preclinical evidence.
06 Treatment decision A prioritized strategy, alternative options, tests still needed, and clinical-trial opportunities.
§ 05 · Frequently asked questions

Before you ask.

Is MENTHIS an NGS laboratory?

No. MENTHIS is a clinical decision-support engine that interprets molecular results produced by qualified laboratories. It does not perform sequencing — it structures the meaning of sequencing data.

Does MENTHIS replace the treating oncologist?

No. MENTHIS is a molecular-oncology decision support tool for physicians and multidisciplinary teams. It never substitutes for the pathology diagnosis, the treating physician's judgment, or multidisciplinary tumor-board review.

If my NGS report is negative, can MENTHIS still help?

Yes. A negative report can stem from limited panel coverage, missing RNA testing, or biomarkers like MSI/TMB/CNV never having been assessed. MENTHIS systematically checks for these possible gaps.

Does MENTHIS identify clinical trials?

Yes, when included in the selected service tier. Clinical-trial matching is performed in a structured way, based on tumor type, molecular alteration, line of therapy, and eligibility logic.
